Work Environment

This position is onsite, and the work primarily occurs in patients' residences throughout South River and surrounding areas. The flexibility of choosing your own hours, within a Monday to Friday workweek, empowers you to manage both professional and personal commitments without compromising on the quality of care provided.

Key Responsibilities

• Conduct thorough evaluations of patients in a home care setting, assessing their physical conditions and rehabilitation needs.
• Develop and implement individualized treatment plans utilizing various modalities, including targeted exercises, hands-on techniques, and equipment to reduce pain and increase mobility.
• Monitor patient progress diligently, documenting treatments and outcomes in compliance with legal and organizational standards.
• Modify treatment goals based on reevaluation and patient progress, adjusting plans to ensure ongoing effectiveness in therapy.
• Recommend and provide training on assistive devices to enhance patient independence in daily activities and ambulation.
• Collaborate with caregivers and family members by providing education on therapeutic techniques and patient care instructions, fostering a supportive recovery environment.
• Stay current on the latest trends and advancements in physical therapy through professional development opportunities and participation in related workshops.

Patient Population / Client Focus

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• State licensure as a Physical Therapist, and certification in CPR.
• Excellent communication skills, both written and verbal, and proficiency in basic computer applications.
• Strong organizational skills and the ability to manage multiple patient care responsibilities simultaneously.
• Physically agile, capable of performing manual therapy techniques with patients.

Preferred Experience

• A minimum of two years' experience as a physical therapist in a clinical or home care setting.
