Work Environment

This role is primarily performed onsite, with a focus on delivering in-home care. You will be working directly in patients' residences, allowing for an intimate perspective on their daily lives and facilitating personalized treatment plans. Key Responsibilities

• Conduct thorough assessments to identify the objectives and specific requirements of each patient, ensuring individualized care plans.
• Manage a patient caseload efficiently, typically seeing one patient per hour, allowing ample time for quality treatment and care.
• Implement evidence-based treatments that are both innovative and cost-effective, focusing on improving clients' motion and mobility.
• Collaborate closely with other healthcare professionals, such as physicians and occupational therapists, to enhance the quality of patient care and coordinate effective interventions.
• Regularly evaluate treatment effectiveness, monitor progress, and adjust care plans as needed to optimize outcomes.
• Maintain comprehensive, up-to-date documentation of patient evaluations, progress notes, and treatment plans in compliance with healthcare regulations and standards.

Qualifications

Compensation and Benefits
