Work Environment

This position is based onsite in a home care setting, allowing therapists to directly engage with patients in their own environments. Key Responsibilities

• Understand state practice acts and adhere to the laws governing the practice of physical therapy, including supervision of other staff as permitted.
• Conduct thorough examinations and evaluations of patients to develop personalized treatment plans that address their functional limitations and disabilities.
• Provide comprehensive evaluation services including reviewing medical histories, performing relevant assessments, and documenting recommendations while maintaining clear and consistent communication with patients and their families.
• Implement treatment protocols based on the evaluation findings tailored to the patient's unique presentation and adjustments within the evolving healthcare landscape.
• Collaborate and consult with other healthcare professionals as necessary, documenting changes in patients' conditions that may require modifications to their plans of care.
• Engage in detailed discharge planning and follow-up care, ensuring accurate documentation of patient progress, including initial evaluations, daily notes, interim summaries, and discharge evaluations.
• Complete ongoing training required by corporate policies and maintain compliance with all state, local, and federal regulations relevant to practice.
• Attend facility meetings, contributing to discussions that aim to remove barriers to patient access and improve service delivery.
• Actively participate in Access Control's initiatives to continuously enhance patient care and workplace efficiency.

Patient Population / Client Focus

You will primarily work with patients receiving home care services who require physical therapy due to a range of conditions, including post-operative recovery, chronic pain issues, and rehabilitation from sports injuries. Your expertise will be instrumental in developing tailored care plans that meet the unique needs of each patient, facilitating their healing process within the comfort of their homes.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program.
• Must hold a valid state license as a Doctor of Physical Therapy (DPT).
• A minimum of two (2) years of outpatient orthopedic experience is required.
• Current CPR certification is mandatory.

Preferred Experience

Experience working in home health settings and proficiency in utilizing electronic medical record (EMR) systems to document patient care is highly preferred. Knowledge of telehealth practices can also be beneficial in adapting care to meet patient needs promptly.

Compensation and Benefits

The salary for this position ranges from USD 100,000 to 120,000 annually, complemented by competitive benefits including:
