Work Environment

This full-time position is set in a variety of home environments throughout the surrounding areas, emphasizing a personalized approach to therapy. You will work primarily Monday through Friday, providing direct treatment and consultation as part of a collaborative healthcare team. Key Responsibilities

• Direct comprehensive physical therapy treatment aimed at preventing disability, relieving pain, improving muscle function, and maximizing client performance as outlined in individualized care plans.
• Evaluate patients' functional levels through diagnostic tests assessing muscle, nerve, joint, and overall functional abilities.
• Conduct home environmental assessments to identify potential barriers to rehabilitation and make recommendations to enhance client independence.
• Supply prescribed physical therapy services in clients' homes, collaborating closely with physicians to ensure continuity of care.
• Implement therapeutic exercises, instructing and motivating clients during manual and non-manual activities to promote recovery.
• Administer traction, massage, and other physical agent treatments to aid rehabilitation efforts.
• Evaluate, fit, and adjust prosthetic and orthotic devices, making modifications as necessary based on clinical assessments.
• Document clinical records, including initial evaluations, care plans, progress notes, and discharge summaries, ensuring accurate and timely submissions.
• Participate actively in interdisciplinary team discussions to develop, review, and refine comprehensive care strategies.
• Educate clients and their families on physical therapy techniques, devices, and overall therapy programs.
• Supervise Physical Therapy Assistants in their duties, ensuring compliance with state regulations and optimal patient care.
• Complete on-site functional assessments and revise care plans in coordination with the client's needs.
• Maintain accessible communication with team members and clients throughout the treatment process.
• Fulfill additional duties as assigned to support the organization in reaching its objectives.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited program.
• One year of relevant experience as a physical therapist, preferably in an acute care or home care setting.
• Active physical therapy licensure in good standing in the state of New Jersey.
• Current CPR certification.
• Excellent interpersonal communication skills, demonstrating the ability to engage effectively with patients and families.
• A strong commitment to clinical excellence and integrity.
• Ability to work collaboratively within an interdisciplinary team.
• Dynamic problem-solving skills and the capacity to lead within patient care initiatives.

Preferred Experience

• Experience working in home care settings.
• Proficiency in using electronic health record (EHR) systems for documentation and patient management.

Compensation and Benefits

This position offers a competitive salary range of $100,000 to $120,000 annually, depending on experience and qualifications.
