Work Environment

Your role will be situated in a welcoming outpatient office environment in South River, New Jersey. Operating Monday through Friday from 8:00 AM to 5:00 PM, you will engage with a diverse patient population, providing high-quality therapy services in a fully equipped facility. Key Responsibilities

• Conduct comprehensive evaluations of patients with functional limitations and disabilities to establish individualized treatment plans based on assessment findings.

• Implement evidence-based treatment protocols while adjusting approaches to align with each patient's evolving condition and needs.

• Document treatment interventions and progress accurately and timely, ensuring all records reflect the patient's care trajectory from initial evaluation through discharge.

• Engage with other healthcare professionals regularly to provide holistic care and recognize when to modify treatment plans or seek additional medical consultations.

• Facilitate effective discharge planning and follow-up care to ensure continuity of treatment that supports long-term patient wellness.

• Participate actively in team meetings, rounds, and training to enhance your skills and contribute to departmental growth and success.

• Remain compliant with state regulations and complete all necessary training and assessments required by corporate and clinical governance.

Patient Population / Client Focus

As a physical therapist at Trails Physical Therapy, you will work with individuals facing a variety of orthopedic and neurological challenges. Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.

• Valid physical therapy license issued by the state of New Jersey.

• A minimum of 2 years of clinical experience in physical therapy, preferably within an outpatient setting.

• Current CPR certification, with the ability to perform basic life support when necessary.

• Strong interpersonal skills with the ability to build rapport quickly with patients and their families.

Preferred Experience

Compensation and Benefits
