Work Environment

As a Home Care Physical Therapist, you will primarily work onsite in various patient homes across Old Bridge and surrounding areas. This full-time position entails a standard schedule of 40 hours per week, with potential for variable weekend hours as needed. You will operate in a fully supportive environment, providing the flexibility of home health care while ensuring patients receive personalized therapeutic services tailored to their needs.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions, functional limitations, and disabilities to develop tailored treatment plans that promote progress and recovery.
• Implement treatment plans that may include therapeutic exercises, mobilization techniques, and functional training aimed at restoring movement and enhancing quality of life.
• Document patient evaluations, treatment sessions, and progress in accordance with legal, regulatory, and professional standards, ensuring accurate and timely billing for services rendered.
• Interact effectively with patients of diverse ages and cultural backgrounds, maintaining the highest standards of customer service throughout the therapy process.
• Collaborate closely with interdisciplinary teams, including physicians and healthcare staff, to ensure continuity of care while addressing patients' needs across different service areas.
• Participate in community outreach and educational initiatives to enhance awareness of physical therapy services and foster collaborations within the community.
• Continuously pursue professional development opportunities through workshops, in-service training, and further education to maintain and enhance clinical skills and knowledge.

Patient Population / Client Focus

Our Home Care Physical Therapists work with a variety of patients, including those recovering from surgery, managing chronic conditions, or dealing with postacute rehabilitation needs. We serve individuals across the lifespan, from children to the elderly, ensuring that our therapy approaches are customized to align with the unique preferences and requirements of each patient, promoting empowerment and independence in their recovery journeys.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• Current state license in Physical Therapy is mandatory.
• Basic Life Support (BLS) certification must be maintained.
• Exceptional oral and written communication skills, with proficiency in electronic medical documentation.
• Ability to demonstrate effective therapeutic techniques and adapt approaches to meet individual patient needs.

Preferred Experience

Candidates should have a minimum of two years of clinical experience in physical therapy, preferably in home health settings or outpatient care. Experience working with diverse populations and familiarity with home health regulations and documentation standards is highly regarded.

Compensation and Benefits

The salary for this position ranges from USD $100,000 to $120,000 per year, commensurate with experience and qualifications.
