Position Overview

Stone Crest Physical Therapy is actively seeking a dedicated and skilled Full-Time Home Care Physical Therapist to join our team in South River, New Jersey. Work Environment

This position is primarily conducted onsite, focusing on home care services. You will engage with patients in their residences across South River, which allows for a unique one-on-one interaction that fosters strong relationships between therapists and patients. Your weekly schedule includes 40 hours, with variable weekends depending on patient needs. Our collaborative approach means you will regularly communicate with a team of healthcare professionals to ensure comprehensive care for our patients.

Key Responsibilities

As a Physical Therapist at Stone Crest, your day-to-day responsibilities will include:

• Conducting thorough evaluations of patients to determine functional limitations and disabilities.
• Developing and implementing individualized treatment plans that align with patient goals and promote optimal recovery.
• Providing expert consultation to healthcare professionals regarding the ongoing care and rehabilitation strategies for patients.
• Conducting regular progress assessments and adjusting treatment plans based on patient responses and feedback.
• Documenting all treatment activities accurately, including the evaluation findings and charges for services rendered.
• Engaging in interdisciplinary collaboration with physicians, families, and other health professionals to enhance patient outcomes.
• Leading community outreach initiatives, contributing to marketing efforts, and participating in continuous quality improvement (CQI) projects.
• Supervising assigned support staff and providing education to interns, students, and clinicians on best practices in physical therapy.

Patient Population / Client Focus

In this role, you will work closely with a diverse patient population, including individuals recovering from surgery, managing chronic conditions, or rehabilitating from injuries. Your expertise will be critical in assessing their unique needs and creating tailored treatment strategies that empower them to regain independence and enhance their quality of life.

Qualifications

To be considered for this position, candidates must possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid Physical Therapy license in the state of New Jersey.
• Strong verbal and written communication skills to effectively engage with patients and collaborate with the healthcare team.
• Proficient computer skills, including experience with electronic health records.

Preferred Experience

Candidates with at least 2 years of experience in a clinical setting are strongly preferred. Familiarity with home health care environments is a plus, as is experience with interdisciplinary care models.

Compensation and Benefits

The compensation for this full-time Physical Therapist position ranges from USD 100,000 to 120,000 annually, commensurate with experience.
