Work Environment

This full-time position is onsite, primarily involving home care settings where you will visit patients in their residences across the Old Bridge area. Our supportive and collaborative environment empowers our Physical Therapists to utilize their skills fully. You'll engage regularly with a multidisciplinary team, including occupational therapists, speech therapists, and rehabilitation specialists, while adhering to best practices and regulatory standards in physical therapy.

Key Responsibilities

• Conduct comprehensive evaluations by reviewing patients' medical history, performing specific assessments, and documenting results and recommendations efficiently.
• Communicate evaluation findings and individualized treatment plans to patients and their families, ensuring they are involved in the goal-setting process.
• Design and implement treatment protocols tailored to each patient's needs and conditions, adapting methods as appropriate based on their progress and any changes in their health status.
• Collaborate with fellow healthcare providers to enhance patient outcomes, recognizing and documenting any changes that may necessitate updates to the rehabilitation plan.
• Develop and execute discharge plans that include follow-up care strategies, ensuring timely and accurate documentation that reflects the patient's treatment course.
• Maintain effective time management practices to meet productivity expectations while ensuring high-quality care delivery.
• Participate in continuous improvement initiatives and team efforts to remove barriers to patient access and enhance the overall patient experience.
• Engage in community outreach and marketing activities, promoting services and increasing awareness of physical therapy benefits.
• Take the initiative in pursuing ongoing professional education and development, serving as a mentor for students or new team members.

Qualifications

• Accredited Doctorate degree in Physical Therapy (DPT) is required.
• Current and valid DPT license to practice in New Jersey.
• American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers is essential.
• State licensure is mandatory as per your work location requirements.

Preferred Experience

We value previous experience in outpatient or home care settings, with a minimum of 2 years working as a physical therapist preferred. Strong interpersonal skills and the ability to collaborate effectively in a team-driven environment will be essential for success in this role.

Compensation and Benefits

The annual salary for this position ranges from USD 100,000 to 120,000, commensurate with experience, along with full benefits.
