Work Environment

This role will primarily focus on home care, allowing you to connect with patients in their most comfortable environment. Key Responsibilities

• Manage the complete therapeutic care process for an assigned group of patients, ensuring a coordinated approach in line with physician directives.
• Conduct thorough patient assessments, integrating findings into personalized care plans to promote goal achievement and enhance patient engagement.
• Deliver or oversee all physical therapy interventions, helping patients navigate their recovery journey effectively.
• Provide detailed evaluations and implement treatment plans adhering to professional ethical standards and legal scope of practice.
• Assess patients' rehabilitative and functional statuses, contributing insights toward developing an all-encompassing care strategy.
• Administer skilled therapeutic care directly within the patient's home, utilizing established procedures to impact their health condition positively.
• Educate patients and their families about therapeutic exercises, the use of any required equipment, and strategies for sustained improvement outside of therapy sessions.
• Prioritize and manage caseloads, determining necessary staffing needs and ensuring seamless care transitions.

Patient Population / Client Focus

Your role will encompass a diverse patient population, ranging from children to older adults, each with unique therapeutic needs. Your ability to personalize care will be crucial in managing conditions related to mobility, strength, and overall functional capabilities. Building strong relationships with both patients and their families will be integral, as your guidance will play a pivotal role in their recovery efforts.

Qualifications

• Current licensure as a Physical Therapist in the state of New Jersey.
• A Bachelor's Degree in Physical Therapy. Preference will be given to candidates with a Doctor of Physical Therapy degree (DPT).
• Basic Cardiac Life Support (BCLS) certification must be obtained within the first 30 days of employment.
• A positive, patient-centered attitude with a focus on delivering exceptional care.

Preferred Experience

While a minimum of one year of clinical experience as a Physical Therapist is desired, qualified new graduates who demonstrate a strong desire to excel in home care settings are encouraged to apply. Experience in managing a diverse patient demographic will be beneficial.

Compensation and Benefits
