Work Environment

This role is set in Freehold, NJ, known for its vibrant community and close-knit atmosphere, where you will work with a supportive team of healthcare professionals. The position requires onsite presence as our physical therapists directly engage with patients in their homes, delivering personalized care that facilitates recovery and fosters independence. A typical workweek runs Monday through Friday, with flexible scheduling to meet patients' needs.

Key Responsibilities

• Conduct comprehensive evaluations, including detailed history reviews, physical assessments, and functional testing to formulate accurate diagnoses and customized treatment plans.
• Communicate evaluation findings and treatment strategies clearly with patients and their families, actively involving them in goal-setting to ensure their engagement in the rehabilitation process.
• Implement tailored treatment protocols that are responsive to individual patient needs and compliant with the evolving healthcare landscape, utilizing the latest evidence-based practices.
• Collaborate with physicians and other healthcare providers to optimize treatment outcomes, adapting plans based on ongoing assessments and patient progress.
• Execute thorough discharge planning and follow-up care, ensuring accurate documentation of each patient's course of care, including initial evaluations and progress notes.
• Manage schedules effectively to maintain productivity goals, participate in meetings punctually, and engage in community outreach through marketing initiatives.
• Adhere to departmental goals while complying with regulatory standards, contributing to the overall quality and performance of the physical therapy department.
• Proactively pursue continuing education opportunities to enhance clinical skills and mentor students or junior staff as a clinical instructor.
• Support initiatives aimed at improving patient access and experiences, actively participating in projects designed to remove barriers to care.

Patient Population / Client Focus

Our orthopedic physical therapists cater to a diverse patient population that includes individuals recovering from sports injuries, surgical interventions, and chronic musculoskeletal conditions. You will work closely with patients of varying ages and backgrounds, customizing your approach to meet their unique needs while fostering trust and motivation throughout their rehabilitation journey.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program.
• Valid state license as a Doctor of Physical Therapist (DPT) issued by the Physical Therapy Examining Committee.
• A minimum of two years of relevant clinical experience in orthopedic rehabilitation.
• Current CPR certification to ensure patient safety during all treatment procedures.

Preferred Experience

Prior experience working in a home care setting is highly preferred. Familiarity with industry-standard documentation practices and a strong grasp of contemporary orthopedic rehabilitation techniques will enhance your effectiveness in this role.
