Work Environment

This position requires the Physical Therapist to work in patients' homes, providing therapy in the comfort of familiar surroundings. As part of our home care team, you will collaborate closely with a diverse group of professionals including doctors, nurses, and allied health staff, ensuring a well-coordinated approach to care.

Key Responsibilities

• Direct and oversee physical therapy treatment plans focused on preventing disability, alleviating pain, and restoring functional mobility according to established care protocols.
• Supervise Physical Therapy Assistants in the implementation of treatment plans while ensuring adherence to clinical best practices and state regulations.
• Conduct thorough evaluations of patient functions through diagnostics that assess muscular, neurological, and joint abilities, assessing needs for adaptive devices as required.
• Evaluate the home environment to establish recommendations that promote the patient's independence, including safe access and mobility within their home.
• Deliver prescribed physical therapy services directly in the home, including therapeutic exercises and hands-on treatment modalities such as traction and massage.
• Collaborate with physicians to devise and update individual care plans based on ongoing evaluations of the patient's progress and condition.
• Prepare comprehensive clinical documentation including initial evaluations, progress notes, and discharge reports while ensuring compliance with OASIS requirements.
• Participate actively in interdisciplinary team meetings, contributing to a holistic treatment approach for each patient.
• Provide education and guidance to patients and their families regarding the use of therapy equipment and adherence to prescribed rehabilitation programs.
• Maintain accurate and timely documentation for clinical records and inform team members about any significant changes in patient health status.
• Take on additional duties as assigned to support organizational goals and improve patient outcomes.

Patient Population / Client Focus

Our clientele typically includes patients recovering from orthopedic surgeries, those with chronic pain conditions, and individuals requiring rehabilitation following injuries. The nature of home care allows us to provide customized therapy tailored to the unique dynamics of each patient's living environment.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited educational institution.
• Current state licensure as a Doctor of Physical Therapy (DPT) from the Physical Therapy Examining Committee.
• At least two years of relevant experience in a clinical setting, with a focus on orthopedic rehabilitation.
• A current CPR certification to ensure the safety and well-being of our patients.

Preferred Experience

• Experience in home health care settings is highly desirable, enabling you to work autonomously and effectively in varied residential environments.
• Familiarity with adaptive equipment and modalities specific to orthopedic rehabilitation.
• Strong communication and interpersonal skills to foster trusting relationships with patients and their families.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience and qualifications. Our benefits package includes:
