Work Environment

This full-time position entails providing therapeutic services onsite in patients' homes. Working in a home care setting allows for an intimate and focused delivery of therapy, ensuring that the treatment is customized to the patient's living environment. Key Responsibilities

• Conduct comprehensive assessments of patients to determine individual needs for physical therapy—this includes evaluating patient history, physical capabilities, and rehabilitation goals.
• Design, implement, and modify personalized treatment plans addressing various conditions, largely in orthopedic, pediatric, neurologic, and sports medicine specialties.
• Employ advanced manual therapy techniques and therapeutic exercises to optimize the rehabilitation process.
• Utilize software tools for patient documentation and treatment tracking, ensuring compliance with HIPAA regulations and maintaining accurate records.
• Provide education and training to patients and their caregivers, empowering them with knowledge about their conditions and rehabilitation processes.
• Meet and exceed minimum utilization and productivity goals while contributing to the organization's business objectives through effective communication and teamwork.

Patient Population / Client Focus

As part of your role, you will engage with a wide array of patients, including those recovering from surgeries, sports injuries, and chronic pain conditions. Qualifications

• Bachelor's degree in Physical Therapy from an APTA or CAPTE-approved program.
• Active state license in Physical Therapy and Basic Life Support Certification from the American Heart Association.
• Demonstrable written and verbal communication skills with an ability to connect with patients of all backgrounds.
• Proficient in computer skills including word processing and relevant therapy software.
• Passion for patient care, combined with the ability to establish lasting relationships that help patients meet or exceed their functional goals.

Preferred Experience

• A minimum of two years of clinical experience in a home care environment or equivalent setting.
• Experience working with a multidisciplinary team to achieve optimal patient outcomes.
• Doctorate of Physical Therapy (DPT) from an accredited program is highly preferred.
• Current CPR certificate to ensure patient safety.

Compensation and Benefits
