Work Environment

This role is primarily based onsite, focusing directly on providing physical therapy in clients' homes. As a member of our team, you will have the flexibility of a Monday through Friday schedule, allowing for a balanced work-life dynamic. You will work closely with Physical Therapy Assistants and other healthcare workers to develop and implement comprehensive treatment plans.

Key Responsibilities

• Direct physical therapy treatment to prevent disability, relieve pain, and restore muscle function in patients based on their individual care plans.
• Conduct evaluations of clients' functional levels through diagnostic tests, tailoring therapy to enhance their independence.
• Collaborate with physical therapy assistants to ensure effective execution of treatment plans, providing guidance and supervision as needed.
• Assess and recommend modifications to the home environment to improve rehabilitation outcomes.
• Administer therapeutic exercises and instruct clients and their families on proper techniques for exercises and the use of physical therapy devices.
• Maintain meticulous clinical records, including initial evaluations, care plans, progress notes, and discharge summaries, ensuring compliance with industry standards.
• Actively participate in interdisciplinary team meetings, bringing insights from your evaluations to contribute to comprehensive patient care discussions.
• Monitor and document changes in clients' conditions and communicate effectively with the interdisciplinary team and physicians regarding any necessary adjustments to treatment plans.
• Provide education to clients and families about the nature of their conditions and encourage adherence to therapy regimens.
• Complete other assignments as directed, contributing to the overall objectives and quality of care within the organization.

Patient Population / Client Focus

You will work with a varied clientele, including older adults and individuals recovering from surgery or physical injuries. The home care focus means that you will tailor interventions to suit patients' home environments, encouraging their involvement and enhancement of their daily activities. Understanding the unique challenges each patient faces in their home setting is crucial for this role.

Qualifications

• A Doctorate in Physical Therapy (DPT) is preferred, though candidates with a Bachelor's Degree in Physical Therapy and relevant experience will also be considered.
• Minimum of one year of relevant clinical experience in a physical therapy setting, preferably in home care or acute care.
• Current and active Physical Therapist licensure in New Jersey.
• CPR certification must be maintained throughout employment.
• Demonstrated commitment to clinical excellence, quality care, and patient-centered practices.
• Strong critical thinking skills, adaptability, and accountability in decision-making processes.
• Exceptional interpersonal communication skills to engage with clients and their families effectively.

Preferred Experience

Compensation and Benefits

This position offers an attractive salary range of USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
