Key Responsibilities

• Conduct comprehensive evaluations of patients recovering from injuries, diseases, or surgeries in their home environments to inform treatment planning.
• Develop and implement individualized treatment plans using a variety of evidence-based methods, such as therapeutic exercises, hands-on techniques, and mobility enhancement strategies.
• Monitor and document the patient's progress by employing various tracking systems, ensuring accurate record-keeping that reflects changes in patient response and improvements.
• Reassess therapeutic goals and adjust treatment plans accordingly to maximize patient recovery and functionality.
• Provide education and training to patients, their families, and caregivers on self-care techniques, mobility assistance, and the use of adaptive devices, ensuring safe and effective home care practices.
• Stay current with developments in physical therapy practices by participating in workshops, continuing education, and professional networking, thereby contributing to your own professional growth and the overall excellence of our team.

Qualifications
