Key Responsibilities

• Conduct comprehensive evaluations to assess patient needs and establish individualized treatment plans based on physicians' referrals.
• Implement and adapt evidence-based therapeutic interventions to facilitate recovery across diverse patient populations, including orthopedic, pediatric, neurologic, and sports medicine cases.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Deliver high-quality care through skilled therapeutic techniques and patient education methodologies.
• Engage actively in consultations with patients and their caregivers, providing essential information and instruction on therapeutic practices to ensure effective home care.
• Maintain accurate documentation of patient interactions and treatment outcomes in accordance with organizational policies.
• Achieve minimum utilization goals and contribute to broader business objectives while maintaining professional standards.

Patient Population / Client Focus

You will be working with a diverse group of patients, tailoring your approach based on their unique needs and goals. This role involves developing meaningful relationships with your patients, focusing on their physical rehabilitation and overall well-being, whether they are recovering from injuries, managing chronic conditions, or seeking to improve their functional capabilities.

Qualifications

• Bachelor's Degree in Physical Therapy or related field.
• Current state license to practice Physical Therapy in New Jersey.
• Basic Life Support (BLS) Certification from the American Heart Association.
• Strong verbal and written communication skills for effective interactions with patients and families.
• Basic proficiency in computer applications and documentation systems related to physical therapy.

Preferred Experience

• Previous experience in a home care environment or outpatient clinical settings.
• Familiarity with evidence-based practice methodologies in physical therapy.
• Experience in managing diverse patient populations and adapting treatment strategies accordingly.
• A proven ability to build rapport and maintain relationships within the community.
