Work Environment

This position is primarily an onsite role, requiring travel to various patient homes within Perth Amboy and surrounding areas. The Home Care setting allows for comprehensive patient engagement in a familiar environment, which can aid in their comfort and recovery. You will be working within a supportive interdisciplinary team, emphasizing collaboration with various healthcare providers to optimize patient outcomes.

Key Responsibilities

• Conduct thorough examinations and evaluations to develop customized treatment plans for patients suffering from functional limitations and disabilities.
• Implement therapeutic interventions, re-evaluate patient progress, and adjust treatment as necessary to achieve optimal results.
• Provide guidance and consultations to healthcare providers regarding a patient's continuum of care.
• Maintain meticulous documentation of evaluations, treatment plans, and progress notes in compliance with all applicable laws and standards.
• Engage in effective communication with patients and their families, educating them on their care and facilitating discharge planning.
• Participate in community outreach, quality improvement initiatives, and marketing efforts to promote the services offered.
• Supervise and mentor support staff in best practices, ensuring a high standard of patient care is maintained.
• Attend staff meetings and professional development sessions to stay updated on the latest healthcare practices and trends.

Patient Population / Client Focus

As a Physical Therapist with Atrium, you will primarily focus on a diverse patient population that includes individuals recovering from orthopedic surgeries, stroke, and various neurological conditions, as well as those with chronic pain or mobility issues. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required.
• A valid State License in Physical Therapy is mandatory to practice in New Jersey.

Preferred Experience
