Work Environment

This role will be primarily onsite, delivering care directly within clients' homes across Woodbridge and surrounding areas. Key Responsibilities

• Conduct comprehensive evaluations for each patient, including detailed assessments of their medical history, physical condition, and rehabilitation needs.
• Develop and communicate individualized treatment plans in coordination with patients and their families, incorporating their goals into the therapeutic process.
• Implement tailored treatment protocols, utilizing techniques that align with the latest research and best practices in physical therapy.
• Regularly reassess patients' progress, making necessary adjustments to treatment plans based on observed outcomes and patient feedback.
• Document all patient interactions meticulously to reflect progress, treatments given, and any modifications made to therapeutic plans in compliance with regulatory standards.
• Collaborate with other healthcare professionals to ensure a multidisciplinary approach to patient care, consulting as needed to facilitate comprehensive rehabilitation.
• Participate in discharge planning, ensuring patients are equipped with the resources and knowledge needed for continued success post-therapy.
• Maintain an organized schedule and time management practices, prioritizing patient appointments and follow-ups to meet productivity expectations.
• Engage in community outreach programs and marketing initiatives to promote awareness of our services.
• Pursue continuing education opportunities to enhance professional skills and knowledge, and serve as a mentor to junior therapists or students.

Patient Population / Client Focus

Your work will directly impact a diverse range of patients, including those recovering from surgery, managing chronic pain, or rehabilitating from sports injuries. You will develop strong relationships with clients and their families, guiding them through their rehabilitation journeys in a compassionate and supportive manner. The flexibility of home care allows you to create personalized strategies, tackling unique challenges as each case requires.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution is mandatory; a Doctorate degree in Physical Therapy (DPT) is preferred.
• Current state licensure to practice physical therapy in New Jersey.
• Certification in Basic Life Support (BLS) from the American Heart Association.

Preferred Experience

• A minimum of 2 years of experience in a physical therapy setting is required, with a strong preference for home care experience.
• Proven ability to develop and implement effective physical therapy treatment plans for a variety of patient needs.
• Excellent interpersonal and communication skills to effectively collaborate with patients, families, and multidisciplinary teams.
