Work Environment

This position is primarily onsite, involving direct patient interaction within a home care setting. Therapists will travel to patients' homes, which allows us to provide personalized care that accommodates each client's circumstances and lifestyle. Working with various patient demographics, physical therapists at Revive utilize advanced techniques and tools to enhance recovery processes. The schedule is designed to support work-life balance, operating Monday to Friday from 9 am to 5 pm.

Key Responsibilities

• Conduct comprehensive evaluations to assess the physical condition and functional abilities of patients, gathering detailed medical histories and setting specific rehabilitation goals.
• Create and implement personalized treatment plans that reflect the assessment findings, ensuring strategies align with each patient's rehabilitation objectives and personal circumstances.
• Utilize hands-on techniques such as manual therapy, therapeutic exercises, and state-of-the-art modalities to ease pain, improve mobility, and support the healing process.
• Educate patients and their families on effective body mechanics, posture, and techniques to prevent further injuries, fostering self-management and long-term wellness.
• Regularly monitor patient progress and adjust treatment plans based on continuous assessments and feedback from patients.
• Collaborate with other healthcare providers to maintain a cohesive approach to patient care, ensuring that all professionals involved are aligned on treatment objectives and updates.
• Document all patient interactions meticulously in our electronic health records system, capturing treatment interventions and clinical outcomes to support ongoing care decisions.

Patient Population / Client Focus

At Revive Physical Therapy, you will have the opportunity to work with a diverse range of patients, including those dealing with chronic pain conditions such as back and neck pain, joint issues, post-surgical rehabilitation, and mobility deficits. Qualifications

• A Bachelor's or Master's degree in Physical Therapy from an accredited institution.
• Current licensure to practice as a Physical Therapist in the state of New Jersey.
• Board certification by the American Physical Therapy Association (APTA).
• A minimum of one year of clinical experience in various rehabilitation settings.
• Proficient knowledge of advanced physical therapy modalities and techniques.
• Ability to assess patient needs critically and adjust therapeutic methods accordingly.
• Excellent communication and interpersonal skills for interacting effectively with patients, families, and the healthcare team.

Preferred Experience

Compensation and Benefits
