Work Environment

This role is onsite, providing care to patients in their homes, allowing for a compassionate, individualized approach to rehabilitation. Key Responsibilities

• Conduct thorough assessments of patients' physical health, identifying functional limitations and developing effective treatment plans.
• Implement evidence-based treatment protocols, customizing interventions based on individual patient presentations and progress.
• Document all aspects of patient care, including initial evaluations, treatment plans, daily progress notes, and discharge summaries, ensuring compliance with healthcare regulations.
• Maintain open communication with other healthcare providers to modify plans of care as required, ensuring holistic treatment approaches.
• Engage with patients and their families, providing education on conditions, treatment options, and self-management strategies.
• Participate in ongoing training and development opportunities to stay abreast of best practices and advancements in physical therapy.

Patient Population / Client Focus

As a Physical Therapist at Headstrong, you will primarily work with adult and geriatric populations who experience a range of orthopedic and neurological conditions. Your expertise will help clients recover their mobility and independence following surgeries, accidents, or chronic conditions. Your efforts will be crucial in managing their rehabilitation journey, focusing on both physical recovery and emotional support.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• Current state licensure as a Physical Therapist (PT) in New Jersey, issued by the Physical Therapy Examining Committee.
• At least two years of experience in physical therapy, preferably in a home care or outpatient orthopedic setting.
• Current CPR certification to ensure patient safety during treatment.

Preferred Experience

• Experience with home health technology and electronic health record systems is a plus.
• Hands-on experience working with patients recovering from major surgeries, accidents, or managing chronic pain conditions.
