Work Environment

This position involves working primarily in patients' homes, allowing you to establish meaningful relationships with clients in their own environments. Key Responsibilities

• Assess, evaluate, and develop personalized treatment plans for individual patients, focusing on their functional limitations and disabilities.
• Perform comprehensive initial evaluations that include detailed history reviews and specific assessments to devise effective treatment protocols.
• Implement targeted rehabilitation techniques based on each patient's needs, adapting approaches to suit any changes in their condition.
• Document all aspects of patient care meticulously, ensuring clarity and compliance with state and federal regulations while painting a comprehensive picture of the patient's journey.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure a holistic approach to patient recovery, including identifying when to adjust treatment plans or seek additional consultations.
• Maintain an organized schedule of home visits, ensuring timely and efficient patient follow-ups and discharge planning.
• Engage in facility meetings, providing feedback and insights that contribute to optimizing patient access and care processes.

Patient Population / Client Focus

The Physical Therapist role directly impacts a diverse patient population, including individuals recovering from surgeries, accidents, and chronic conditions. Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited institution.
• Current state license as a Physical Therapist (PT) issued by the Physical Therapy Examining Committee.
• Minimum of two years of outpatient orthopedic experience, with a strong foundation in therapeutic techniques and patient management.
• Current CPR Certification, ensuring readiness to respond to medical emergencies.

Preferred Experience

While we require two years of experience, candidates with advanced clinical expertise in specific therapeutic modalities or prior experience in home health settings will be given preference. Familiarity with electronic health record (EHR) systems to document patient interactions and progress will also be advantageous.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 100,000 to USD 120,000, commensurate with experience. Our comprehensive benefits package includes:
