Work Environment

This position requires on-site work as a Home Care Physical Therapist, with a typical commitment of 40 hours per week, including variable weekends. Key Responsibilities

As a Physical Therapist, your primary responsibilities include:

• Conducting thorough evaluations of patients' functional abilities, limitations, and goals to develop tailored treatment plans.
• Providing expert consultation to healthcare providers to ensure continuity and effectiveness of care for each patient.
• Maintaining compliance with customer service standards while demonstrating empathy and respect when interacting with patients and their families.
• Implementing treatment protocols based on evaluations, continually re-evaluating patient progress and modifying plans as necessary.
• Documenting assessments, treatment sessions, and referrals accurately in electronic health records in adherence to legal and regulatory standards.
• Collaborating closely with multidisciplinary teams, physicians, and referral sources to optimize patient outcomes and ensure seamless care transitions.
• Engaging in community outreach initiatives and participating in program development to enhance department services.
• Staying up-to-date with the latest best practices through continuous education, attending professional seminars, and participating in staff meetings.

Patient Population / Client Focus

You will primarily work with a diverse patient population in Freehold, New Jersey, including those facing orthopedic challenges, rehabilitation post-surgery, or recovering from injury. The practice focuses on personalized care, ensuring that treatment plans are not only effective but also respectful of each patient's unique cultural and social backgrounds.

Qualifications

To be successful in this role, candidates must possess:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid and current state license to practice physical therapy.
• Excellent verbal and written communication skills to effectively engage with patients and healthcare team members.
• Competency in word processing and familiarity with electronic health record systems.

Preferred Experience

Compensation and Benefits

Flex Staff Physical Therapy offers a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
