Key Responsibilities\n

• Prepare and maintain accurate electronic medical records in compliance with departmental and health system policies, ensuring all documentation is thorough for continuity of care.

• Conduct thorough psychosocial assessments of patients and their families upon admission and periodically throughout their treatment, identifying emotional, social, and physical needs while applying relevant social work theories.

• Collaborate with interdisciplinary health care teams, contributing to the design and implementation of individualized psychosocial treatment plans, which include referrals to specialty services and documenting progress notes effectively.

• Provide crisis intervention and supportive counseling services to assist patients and families in coping with emotional challenges arising from illness, while navigating complex medical information.

• Effectively manage an independent caseload that includes varying levels of clinical acuity, ensuring timely responses to patients' needs.

• Identify patients at risk for psychosocial issues through case finding, actively participating in team conferences and interdisciplinary rounds to enhance treatment planning.

• Facilitate psychoeducation sessions designed to help patients and families adjust to diagnoses and treatment plans while navigating transitions between services.

• Engage in quality assurance activities, contributing insights for the continual improvement of social work policies and procedures relevant to our patient population.

• When necessary, perform mental health diagnoses as part of comprehensive treatment plans.

• Provide clinical supervision to MSWs and student interns, fostering professional growth and development within the social work discipline.

Qualifications\n

Compensation and Benefits\n

This full-time position offers a competitive salary range between USD 120,000 and 130,000 per year, commensurate with experience.
