Key Responsibilities\n

• Perform thorough assessments to diagnose and treat a variety of mental, emotional, and behavioral disorders, making necessary modifications to treatment plans based on patient progress.

• Continuously monitor and evaluate patient needs, ensuring timely and effective adjustments to treatment strategies as indications arise.

• Facilitate referrals to allied professionals, such as psychiatrists, support groups, or community services, contributing significantly to the overall well-being of patients.

• Collaborate with a multidisciplinary care team to formulate robust discharge plans, ensuring continuity of care and resources upon patients' return to their communities.

• Comply with all applicable standards set forth by federal, state, and local regulations, actively practicing within the Mental Health Practice Act framework.

• Engage in proactive communication with external agencies and care networks to navigate and resolve obstacles facing patients during their treatment journey.

• Conduct social work consultations for clients and their caregivers while employing a variety of therapeutic approaches, including crisis intervention and bereavement support.

• Advocate for patient needs, helping facilitate clear communication between clients and their support networks.

• Identify patients at risk for readmission or extended stays, coordinating with resources to optimize discharge processes and treatment outcomes.

Qualifications\n

Compensation and Benefits\n

The compensation for this position ranges from $90,000 to $100,000 annually, reflecting your expertise and experience. Additional financial incentives include a sign-on bonus of up to $1,000.
