Summary

Native American Community Clinic opened its doors in 2003 to address the health disparities within the urban Native American community of the Twin Cities. Our mission is to promote the health & wellness of mind, body, and spirit of Native American families. We offer a full range of healthcare services that include Medical, Behavioral Health, Dental, and Substance Abuse programs. NACC approaches healthcare by addressing root causes of health disparities including access to food, housing and health insurance with services such as resource navigation, care coordination, outreach and community-based activities through the use of our peer recovery coaches and community health workers. NACC strives to honor health and tradition by providing spiritual care and access to traditional healing through our Elders in Residence.

The Native American Community Clinic (NACC) is a community health center in Minneapolis dedicated to serving urban Indigenous communities and other underserved groups. We are seeking a compassionate, resourceful, community-oriented Resident Connections Coordinator to join our team. This position plays a vital role in supporting residents with social service needs, including assistance with maintaining safe/stable housing, resources/referrals for children 0-18 yrs of age, health insurance, food security, transportation information, and other essential supports. The Resident Connection Coordinator works closely with internal/ external teams and community partners to remove barriers to care, improve access to resources, and enhance overall patient well-being through culturally responsive, holistic support.

Wihinaphe Apartments, a new affordable housing community located above the new Native American Community Clinic in Minneapolis, opening in September 2026. This housing development is being led by Wellington Management in partnership with Native American Community Clinic (NACC) and will include:

• 83 affordable apartment units • Studio, 1, 2, 3, and 4-bedroom options

• Underground parking • Community-focused amenities

This position is part of a Supportive Housing Services partnership between NACC and Simpson Housing Services, the position will provide supportive services to residents at the Whinaphe Apartment Building for people with disabilities that provides formerly homeless single adults and families with a safe and permanent home and support services to help residents regain and maintain stability in their lives, homes and the community. This energetic and adaptable employee will work as part of a team to assist residents to maintain housing, support access to community resources, and foster community engagement and resident leadership. The Resident Connections Coordinator (RCC) must be knowledgeable about and able to respond effectively to the complexities of participants’ lives and the co-occurring challenges and conditions associated with long-term homelessness.

The RCC will work as part of a team comprised of staff from different agency partners (Simpson, Wellington). Hours for this position are Mondays to Fridays, 9-5pm.

Responsibilities

Provide quality service to participants:

• Welcome new residents and actively build relationships, explaining the resident services program, and offering information and support to those wishing to access resources.

• Develop supportive professional relationships with residents that help enhance the quality of their lives, empower and encourage them in taking the steps to achieve self-sufficiency.

• Identify, develop and maintain referral partnerships with local social service agencies that effectively assist residents to achieve their basic needs and life goals in areas including education, employment, mental and physical health. Accessing mental health crisis services or Adult Protective Services for residents is a common component of the position.

• Partner with Property Management when a resident is in jeopardy of eviction and offer support to the resident to positively and quickly rectify the situation.

• Provide flexible services during times that meet needs of participants.

Provide opportunities for community engagement for residents:

• Facilitate resident meetings with Property Management and implement community engagement activities such as support groups, movie/game nights, resident leadership council, community health education workshops and social celebrations, sometimes with the support of outside volunteers.

• Provide support and leadership to team members to enhance community engagement at Whinaphe.

• Ensure residents are informed of opportunities to participate in the direction of programs, and that residents have opportunity and systematic access for expressing concerns and affecting positive change.

• Encourage and facilitate resident leadership through the RPC (Resident Planning Committee) and other resident leadership opportunities.

Demonstrate teamwork and collaboration skills with 3 different agency partners:

• Communicate and coordinate services with all team members. Attend regular team meetings and other required meetings.

• Develop and maintain positive working relationships with staff, volunteers, participants and other stakeholders.

• Network with other community providers of similar services

Complete accurate and thorough documentation and recordkeeping in a timely manner:

• Maintain individual electronic Client Track files, including timely detailed case notes, signed data privacy, intake and discharge summaries, and Releases of Information.

• Understand and comply with necessary documentation for MN Housing and Hennepin County Housing Support.

• Maintain accurate records including employee time sheets and travel expenses.

• Participate with funding reports, and site visits as requested.

Demonstrate essential knowledge, abilities, and skills:

• Ability to work very well independently, with excellent skills in teamwork and collaboration.

• Ability to be flexible and manage time.

• Knowledge of Harm Reduction/Housing First Model and commitment to that model.

• Knowledge of trauma-informed service and motivational interviewing techniques.

• Keep up to date and knowledgeable on community resources and trends. Expand knowledge base around topics that affect Whinaphe resident population.

• Ability to communicate effectively, positively and professionally with participants, providers, landlords, family members and others.

• Ability to promote a professional work environment that is affirming, respectful and understanding of diverse people.

• Have organizational skills and be able to multi-task to accomplish the day-to-day activities.

• Must possess good computer skills, with experience in Microsoft Word, Excel, and Outlook. Demonstrate ability to learn additional computer skills/program as necessary.

• Ability to maintain confidentiality.

• Adhere to a standard framework of boundaries in all interactions with guests, volunteers, staff and public.

Job Requirements:

• Bachelor’s degree in human services and/or related field of study or equivalent experience is required.

• Experience working with individuals who are homeless or have experienced homelessness.

• Broad knowledge of housing resources.

• Driver’s license and access to a car.

Desired:

• Experience working with adults with mental illness and/or chemical dependency.

Essential Physical Functions

The employee must be able to hear and speak, in order to communicate with staff and participants in person, on the telephone and in writing. Employee must be able to move about the community and public places (such as apartment buildings). Employees must be able to lift items up to 25 pounds on an occasional basis.
