Work Environment

You will be working in our home care facility, which allows for a unique opportunity to provide therapy directly in patients' homes. Monday to Friday scheduling ensures work-life balance while enabling you to manage your caseload efficiently within the community.

Key Responsibilities

• Conduct comprehensive evaluations by reviewing patients' medical histories, performing physical assessments, and documenting the results accurately.
• Communicate evaluation findings and treatment plans effectively to both patients and their families, fostering collaborative goal-setting.
• Implement individualized treatment protocols based on evaluation findings and remain adaptive to each patient's progress and changing conditions.
• Consult with interdisciplinary healthcare professionals to provide optimal care and modify treatment plans when necessary.
• Ensure meticulous documentation of all patient care activities, reflecting the patient's course of care from initial evaluation to discharge.
• Participate in the development of department goals, maintaining compliance with regulatory requirements and organizational standards.
• Actively engage in community outreach and marketing initiatives to promote our services and address barriers to patient access.
• Mentor and guide students and junior staff as a clinical instructor to foster the next generation of therapists.

Patient Population / Client Focus

The primary focus of this role will be on orthopedic patients requiring rehabilitation due to surgical procedures, injuries, or chronic pain conditions. You will support a diverse range of clients, from athletes recovering from sports-related injuries to elderly patients seeking to regain mobility post-surgery. Our goal is to enhance each patient's quality of life through personalized care plans and therapeutic interventions.

Qualifications

• Bachelor's degree in Physical Therapy required; a Doctorate in Physical Therapy is preferred.
• Active state licensing in New Jersey is mandatory to practice.
• Current American Heart Association's BLS for Healthcare Providers CPR certification is a must.

Preferred Experience

• Experience in orthopedic rehabilitation is preferred but not mandatory; we welcome recent graduates eager to learn and grow.
• Previous involvement in home care settings or outpatient clinics will be considered an asset.

Compensation and Benefits
