Key Responsibilities\n

• Conduct in-depth assessments of seniors’ psychosocial needs, factoring in emotional, mental, and social influences on their overall well-being.

• Collaborate with clients and their families to create personalized care plans that reflect individual goals and preferences.

• Provide counseling and emotional support, assisting seniors and families as they navigate transitions such as relocation, bereavement, or health changes.
\n
• Advocate for clients' rights and accessibility to community resources, ensuring they receive comprehensive support and guidance.

• Engage with interdisciplinary teams—comprising nurses, caregivers, and other healthcare professionals—to coordinate care effectively.

• Conduct regular virtual check-ins and home assessments to monitor clients’ well-being, reassess their living conditions, and address any safety concerns.

• Facilitate communication among family members involved in senior care, helping navigate conflicts and promote a collaborative approach to care.
\n
• Educate clients and their families on available community resources, support groups, and programs to enhance social engagement.
\n
• Maintain accurate and timely documentation of client interactions, assessments, care plans, and progress notes in our electronic health record (EHR) system.

• Stay abreast of evolving social work practices, policies, and regulations to ensure adherence to ethical standards.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• A current and valid Clinical Social Worker licensure (LCSW) in the state of Colorado.

• At least two years of relevant experience in a social work role, ideally in community health or a geriatric-focused setting.

• In-depth knowledge of social work principles, aging-related issues, and the community resources available to seniors.

• Exceptional assessment, counseling, and problem-solving skills.
\n
• A strong ability to empathize, nurture, and build rapport with clients and their families.

• Excellent communication and interpersonal skills for effective collaboration with multidisciplinary teams.

• Proficiency in using electronic health record systems and various computer applications for documentation and reporting tasks.

Preferred Experience\n

Compensation and Benefits\n
