Work Environment

The position is primarily onsite, providing care directly in patients' homes. Key Responsibilities

• Perform thorough evaluations of patients to identify functional limitations and disabilities, developing individualized treatment plans based on assessment findings.

• Consult with other healthcare providers regarding patient care continuity, ensuring proper communication and collaboration for optimal outcomes.

Qualifications

• Minimum of a Bachelor's Degree in Physical Therapy from an accredited institution.
