Key Responsibilities\n

As a Licensed Clinical Social Worker at Valley Point, you will have a diverse range of responsibilities, including:

• Conducting comprehensive psychosocial assessments to evaluate patients' emotional, social, and financial needs.

• Providing individual and group counseling sessions, specializing in adjustment challenges, grief, and coping strategies.
\n
• Collaborating with physicians and nursing professionals to enhance patient outcomes through a multidisciplinary approach.

• Coordinating care by connecting patients with community resources post-discharge, ensuring continuity and support during recovery.
\n
• Offering emotional education for patients and families about available treatment options and community resources.
\n
• Participating in training programs and hospital committees to enhance the quality of social work practices.

• Delivering crisis intervention and ongoing counseling to patients and their families, tailored to unique healthcare situations.

• Documenting patient interactions and plans accurately and timely according to hospital policies and legal guidelines.

Patient Population / Client Focus\n

Our patient population includes individuals undergoing detoxification and recovery from substance use disorders. You will provide holistic support for patients facing various psychosocial challenges, including mental health issues, societal reintegration stressors, and economic difficulties. By providing targeted counseling and resources, you will help these individuals navigate their journeys toward recovery.

Qualifications\n

To be considered for this role, candidates must possess the following qualifications:

• A Master's degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).
