Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs through interviews, observations, and collaboration with other healthcare providers.

• Design and implement personalized care plans that address the individual goals and preferences of clients and their families, ensuring that all plans are tailored to meet psychological, social, and emotional needs.

• Provide ongoing counseling and emotional support, helping seniors navigate difficult life transitions such as health crises, loss, or changes in living circumstances.

• Actively advocate for clients’ rights and their access to appropriate community resources, ensuring that they benefit from available support services and programs.

• Document and maintain comprehensive records of client interactions, assessments, and progress reports, utilizing electronic health record systems to manage information efficiently.

• Develop and facilitate workshops or support groups aimed at enhancing social connections among seniors, promoting mental health awareness, and encouraging healthy coping strategies.

Patient Population / Client Focus\n

This role primarily focuses on seniors within our community who may be experiencing a variety of challenges including mental health issues, chronic illness, social isolation, and end-of-life transitions. By evaluating their unique circumstances, you will help implement strategies that encourage independence, dignity, and quality of life for these individuals and their families.

Qualifications\n

• Master's degree in Social Work from an accredited institution is required.

• Must hold a current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of relevant experience in a social work role, ideally within a community center or geriatric setting.

• Strong understanding of social work principles, diverse aging-related issues, and local community resources available to seniors.

• Exceptional interpersonal, assessment, counseling, and problem-solving skills, with a focus on empathetic communication and active listening.

Compensation and Benefits\n
