Work Environment\n

This role is fully remote, allowing for flexibility in managing your time while working closely with a dedicated team committed to hospice care. As part of our Hospice Center, you will collaborate with nurses, physicians, and other health professionals, ensuring a cohesive approach to patient care. Our remote infrastructure is designed to support effective communication and collaboration, utilizing various platforms to stay connected and engaged.

Key Responsibilities\n

• Conduct thorough evaluations and assessments of patients to determine their social, emotional, and psychological needs.
\n
• Collaborate with a multidisciplinary team to develop and implement individual care plans in alignment with the patient’s wishes and goals.
\n
• Facilitate Individualized Family Service Plan (IFSP) development and implementation, ensuring active participation from family members.
\n
• Monitor and track patient outcomes, adjusting care plans as necessary to ensure optimal support.
\n
• Provide emotional support and counseling to patients and their families, helping them cope with the challenges of terminal illness.
\n
• Educate families on available resources, tools, and coping strategies to better manage their loved one's care.
\n
• Maintain compliance with all Federal and State regulations regarding hospice care.
\n
• Utilize evidence-based practices and natural learning environment methods to enhance patient and family engagement.
\n
• Foster rapport with patients, families, and team members to create a supportive and empathetic atmosphere.

Patient Population / Client Focus\n

In this role, you will be working with patients who are in hospice care, often at the end stages of life, as well as their families. Patients may present with a wide array of complex emotional needs due to terminal illness, necessitating a skilled and empathetic approach. You will play a crucial role in advocating for patient wishes and supporting families as they navigate sensitive end-of-life decisions, believing in the dignity and comfort of each individual.

Qualifications\n

• Bachelor's degree in Social Work (BSW) required; a Master’s Degree or Doctorate in Social Work (MSW/DSW) is preferred.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.

• Valid driver’s license and reliable transportation are necessary for visiting patients when applicable.
\n
• Strong knowledge of hospice and palliative care practices, regulations, and therapeutic modalities.

• Excellent organizational, time management, and communication skills, with an emphasis on teamwork.

Preferred Experience\n

• A minimum of one year of experience working in social work, particularly in hospice, palliative care, or relevant healthcare settings.
