Work Environment\n

This role will primarily be performed remotely, allowing for flexibility in managing work-life balance while closely collaborating with a multidisciplinary team of providers. As part of our hospice center, you will engage with families and medical professionals through virtual platforms to deliver critical support and interventions. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments for children in hospice care, identifying their unique strengths and needs.

• Develop and implement Individualized Family Service Plans (IFSP) alongside families, setting measurable goals that align with each child's developmental progress.

• Monitor patient outcomes and provide ongoing support, adjusting interventions and strategies based on real-time assessments.

• Establish rapport with children and families to create a supportive learning and therapeutic environment.
\n
• Utilize coaching models and Natural Learning Environment Practices to deliver effective interventions that enhance developmental outcomes.

• Educate families about their child’s developmental journey, providing actionable strategies and support tailored to individual needs.

• Ensure compliance with all relevant federal and state regulations regarding pediatric hospice care.

Patient Population / Client Focus\n

This role primarily focuses on young children with disabilities or developmental delays within a hospice framework. Our services aim to support infants and toddlers, helping them achieve critical milestones in physical, cognitive, social/emotional, and communication skills during these formative years. By delivering compassionate care and collaborating with families, you will play a pivotal role in improving the quality of life for these children and their loved ones.

Qualifications\n

• Master’s Degree in Social Work or a Doctorate in Social Work.

• Active licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.
