Work Environment

This position requires working onsite within the home care environment, engaging directly with clients in their residences. You will work a standard 40-hour week, which may include rotational weekends to provide comprehensive care. Key Responsibilities

• Conduct detailed assessments of patients with orthopedic conditions to develop personalized treatment plans addressing their functional limitations and rehabilitation goals.
• Implement therapy sessions that utilize a variety of therapeutic techniques, including joint mobilization, manual therapy, and exercise regimens.
• Collaborate with physicians, nurses, and other healthcare providers to coordinate and optimize patient care.
• Document patient progress thoroughly, ensuring compliance with all relevant regulations and quality standards.
• Engage in community outreach initiatives, helping educate the public on injury prevention and the benefits of orthopedic therapy.
• Participate in regular team meetings, professional development opportunities, and continuing education to enhance clinical skills and knowledge.

Patient Population / Client Focus

Your primary focus will be treating adult patients recovering from orthopedic surgeries, injuries, and chronic conditions. This includes working with individuals experiencing pain, mobility limitations, and functional discrepancies related to their orthopedic health. You will be tasked with customizing therapy plans that cater to each patient's unique circumstances, ultimately supporting their journey towards recovery and functional independence.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Active and valid State License for Physical Therapy.
• Basic Life Support (BLS) certification.
• Strong oral and written communication skills for effective patient interactions and documentation.
• Proficiency in using electronic health record systems and standard office software applications.

Preferred Experience

Ideal candidates will have experience working in an orthopedic setting, demonstrating familiarity with various treatment modalities and therapeutic exercises specific to orthopedic rehabilitation. Experience with home health care services and multi-disciplinary teamwork will be regarded favorably.

Compensation and Benefits
