Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify and address emotional, social, and environmental issues impacting patients’ health and recovery.

• Develop, implement, and adjust tailored intervention plans in collaboration with patients, their families, and the healthcare team.

• Facilitate effective discharge planning by working closely with multidisciplinary teams to ensure a seamless transition to post-treatment care.

• Provide education and counseling to patients and family members, focusing on coping strategies and adjustment to life after hospitalization.

• Utilize various therapeutic modalities such as crisis intervention, individual, group, and family therapy to meet diverse patient needs.

• Serve as a knowledgeable resource within the organization, offering guidance to staff and participating in educational workshops as needed.

• Engage with external stakeholders, including consulting physicians and insurance providers, to enhance patient care and system navigation.

• Participate in on-call rotations to support after-hours patient needs and interventions.
\n
• Execute involuntary examinations when clinically necessary in accordance with the Baker Act, ensuring patient rights and safety.

• Contribute to community outreach efforts by providing consultations, participating in research initiatives, and presenting at educational conferences on psychosocial oncology practices.

Patient Population / Client Focus\n

This position primarily serves patients undergoing detoxification from substance use disorders, as well as individuals facing co-occurring mental health challenges. The LCSW will support adults from diverse backgrounds, tailoring approaches to accommodate a wide range of experiences and circumstances. Qualifications\n

• Bachelor's degree in Social Work (required); a Master’s degree is preferred.

Entryway Family & Psychiatric Services offers a competitive salary ranging from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
