Work Environment

The position of Physical Therapist is primarily on-site, where you will work directly in our patients' homes. Key Responsibilities

• Conduct comprehensive evaluations of patients to identify physical impairments, functional limitations, and disabilities stemming from various health conditions.

• Develop individualized treatment programs based on patient assessments and in accordance with physician orders, ensuring timely modifications as needed.

• Maintain detailed records of patient interactions, including care plans, treatment progress, and discharge summaries, using professional documentation standards to justify skilled care.

• Guide and supervise Physical Therapist Assistants (PTAs) and support personnel during treatment sessions to ensure the delivery of high-quality patient care.

• Collaborate with multidisciplinary healthcare professionals to coordinate patient interventions, including regular communication with physicians, nurses, and case managers.

• Stay updated on evidence-based practices and therapeutic technologies to enhance treatment efficacy and improve patient outcomes.

• Actively participate in quality improvement initiatives and outreach programs aimed at improving patient care and engagement.

Patient Population / Client Focus

As a Physical Therapist, you will work with a diverse population, including individuals recovering from injuries, managing chronic conditions, or facing age-related physical limitations. Qualifications

• A Bachelor of Science degree in Physical Therapy from an accredited institution.

• Current licensure as a Physical Therapist in the state of Virginia.

• Competence in using electronic medical record (EMR) systems and basic computer applications for documentation and communication purposes.

Preferred Experience

Compensation and Benefits
