Work Environment

This position is primarily onsite — in the comfort of patients' homes. Working in the home care setting allows our Physical Therapists to observe and address individual needs and circumstances that may affect recovery. You will engage with patients in a variety of residential environments, ensuring they receive quality care tailored to their day-to-day lives. You will also be a vital team member collaborating with other healthcare professionals to enhance patient outcomes.

Key Responsibilities

• Evaluate patients referred by physicians, assess their needs and develop comprehensive, personalized treatment plans aimed at restoring function and improving quality of life.
• Implement and adjust therapy programs for a diverse range of conditions, including orthopedic, pediatric, neurologic, and sports medicine cases.
• Utilize advanced therapeutic techniques to provide care that aligns with established goals and objectives while maximizing patient engagement.
• Document patient progress accurately and timely, ensuring all records comply with clinic protocols and regulatory standards.
• Educate and empower patients and their caregivers on therapy procedures, recommending home exercise and lifestyle modifications to support continued recovery.
• Regularly meet established utilization goals and contribute positively to the overall efficiency of our service delivery.

Patient Population / Client Focus

The primary focus of your role will be to serve a diverse patient population, including individuals recovering from surgeries, sports injuries, neurological conditions, and orthopedic-related issues. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program (APTA or CAPTE approved).
• A valid State License in Physical Therapy to provide care in California.
• Current Basic Life Support Certification from the American Heart Association.
• Strong communication skills, both written and verbal, to foster effective interactions with patients, families, and healthcare team members.
• Proficient in computer skills and electronic documentation systems to ensure accurate and timely patient records.
• Ability to build strong relationships with patients, promoting a compassionate clinical environment that empowers them to exceed their set functional goals.

Preferred Experience

• Prior experience in home health care settings or familiarity with delivering therapy in a patient's residence is highly desirable.
• Experience dealing with a variety of patient populations is advantageous, particularly in pediatrics and neurology.
• Previous involvement in team-based care, including working alongside interdisciplinary health professionals, is preferred.
