Work Environment\n

This position is primarily remote, enabling you to provide services to students from the comfort of your selected workspace while staying engaged with our multidisciplinary treatment teams. The role mandates participation in virtual meetings and occasional travel to school sites and various CHDC locations for meetings, retreats, and outreach events. Key Responsibilities\n

• Deliver clinical behavioral health services in alignment with CHDC’s mission to enhance patient health outcomes and satisfaction.

• Support students and families in navigating the complexities of physical and behavioral healthcare systems to ensure integrated care delivery.
\n
• Equip individuals with information about resources and services available to them, actively promoting wellness, disease prevention, and healthy lifestyle choices.
\n
• Engage in regular multidisciplinary treatment team meetings, sharing insights and strategies for patient care, and contributing to collaborative treatment plans.
\n
• Timely report all crisis situations and unusual incidents to the Medical Director, with immediate escalation of psychiatric and medical crises to the Chief Medical Officer.
\n
• Maintain accurate and up-to-date documentation for each individual in their clinical records, adhering to best practices in record keeping.
\n
• Participate in the Quality Improvement Program, including attending all relevant meetings and training sessions to stay updated on best practices.
\n
• Utilize evidence-based screening tools to assess individual healthcare needs and ensure appropriate interventions while reporting results accordingly.
\n
• Collaborate with our Center of Excellence Team to provide specialized support for patients with Opioid Use Disorder, further enhancing integrated care approaches.

Patient Population / Client Focus\n

The primary focus of this role is on school-aged children and adolescents, providing mental health support tailored to their unique needs. You will work with a diverse population, addressing a variety of behavioral health challenges, including anxiety, depression, and substance use disorders. Your contributions will directly impact the mental well-being of students, enhancing their ability to learn and thrive in a supportive educational environment.

Qualifications\n

• A Master’s degree in Social Work is required, with an active Licensed Clinical Social Worker (LCSW) credential.

• A minimum of 2–3 years of relevant professional experience in social work, particularly within integrated health or behavioral health settings.

• Capability to collaborate effectively with healthcare professionals, social service providers, students, and their families.
\n
• Willingness and ability to travel to various CHDC sites for meetings and community engagement activities.

Preferred Experience\n

• Experience working within a school-based setting or with children and adolescent populations is highly preferred.

• Familiarity with integrated care models and a strong understanding of the challenges faced in addressing both physical and mental health needs.

• Proficiency in utilizing electronic health records and other clinical documentation systems to ensure efficient workflow and compliance with best practices.

Compensation and Benefits\n

This full-time position offers a competitive salary ranging from USD 90,000 to 100,000 annually.
