Work Environment

This position is primarily onsite, providing care in the home environments of our clients. The nature of home care requires adaptability and creativity as Physical Therapists work in diverse settings, each with its own unique challenges and opportunities. As an integral part of our team, you will provide one-on-one therapy in a supportive environment aimed at fostering meaningful patient relationships.

Key Responsibilities

• Direct comprehensive physical therapy treatment plans focused on preventing disability, alleviating pain, and enhancing muscle function.
• Supervise and mentor Physical Therapy Assistants to ensure the effective delivery of prescribed care plans.
• Conduct thorough evaluations of patients' functional levels using advanced diagnostic and prognostic tests related to muscle and joint capabilities.
• Assess and optimize home environments to support patient rehabilitation and maximize independence.
• Perform manual therapy techniques, administer exercises, and guide clients through non-manual activities to promote recovery.
• Implement specialized treatments such as traction and massage, alongside the application of physical agents as part of the therapy plan.
• Fit and adjust prosthetic and orthotic devices, advising on necessary modifications to ensure proper use and functionality.
• Participate actively in the formulations, reviews, and adjustments of individualized care plans, ensuring they reflect the current needs of the client.
• Maintain detailed clinical documentation including initial evaluations, care plans, progress notes, and discharge summaries, meeting regulatory requirements.
• Engage in interdisciplinary collaboration during case conferences to evaluate and enhance comprehensive treatment strategies.
• Provide education and consultation to clients and families about therapy practices and assist them in using prescribed therapeutic devices.
• Respond to and adapt the treatment plans based on observations of client progress and feedback from other care providers.
• Complete all assignments with a commitment to maintaining high standards of care and adhering to company policies.

Patient Population / Client Focus

The Primary focus is on adult and senior clients requiring rehabilitation and management of orthopedic conditions. Our therapists engage with patients recovering from surgeries, injuries, or chronic conditions—ensuring they receive not only the physical support they need but also respect and understanding for their personal health journeys.

Qualifications

• A degree from an accredited physical therapy program.
• Active licensure to practice as a Physical Therapist in Virginia, ensuring compliance with state regulations.
• A minimum of one year of experience in a physical therapy role, particularly in acute care or home care settings.
• Current CPR certification and a valid driver's license with automobile insurance.
• A proactive and creative mindset, capable of troubleshooting to provide exceptional patient care.
• Demonstrated leadership and decision-making abilities, particularly in high-stakes environments.

Preferred Experience

• Previous home healthcare experience is strongly preferred, demonstrating a dynamic approach to patient care in varying environments.

Compensation and Benefits

The annual salary for this position ranges from USD 100,000 to 120,000, based on experience and qualifications.
