Work Environment

This role is an onsite position that operates mainly within patients' homes, allowing you to deliver personalized care in a familiar and comfortable setting. You will be part of a dedicated team of professionals who prioritize patient safety and satisfaction. Your weekly schedule will encompass approximately 40 hours, with variable weekends as necessary to meet patient needs. Key Responsibilities

• Conduct thorough examinations and evaluations of patients to develop tailored treatment plans aimed at addressing their specific functional limitations and disabilities.
• Effectively consult with healthcare providers on a patient's continuum of care to ensure a collaborative approach to treatment.
• Engage with patients of all ages, ensuring adherence to our customer service standards while promoting a nurturing environment.
• Implement and re-evaluate treatment plans regularly, incorporating patient feedback and adjusting therapies as needed.
• Document treatment plans, evaluations, and progress accurately, ensuring compliance with applicable laws, regulations, and professional standards.
• Collaborate with interdisciplinary teams, referral sources, and physicians to maintain seamless communication regarding patient care.
• Participate in ongoing education and training programs to enhance clinical skills and stay current with best practices in physical therapy.
• Manage department resources efficiently, supervising support staff and ensuring the highest quality of service delivery.

Patient Population / Client Focus

Your primary focus will be on patients experiencing orthopedic conditions ranging from post-surgical recovery to chronic pain management. Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A current State License in Physical Therapy is necessary for practice.
• Basic Life Support (BLS) certification is required.
• Strong written and verbal communication skills are essential for effective patient and team interactions.
• Proficiency in word processing and electronic documentation systems is necessary for accurate record-keeping.

Preferred Experience

While not mandatory, previous experience in an orthopedic-focused outpatient setting or home health care may provide candidates with a distinct advantage in effectively addressing the needs of our patient population.

Compensation and Benefits
