Key Responsibilities

• Evaluate patients referred by physicians to create tailored treatment plans aimed at achieving recovery goals.
• Implement evidence-based practices to treat a diverse range of conditions, focusing on orthopedic problems while also accommodating pediatric, neurologic, and sports medicine cases.
• Utilize specialized techniques and technologies to manage patient care effectively, monitoring progress and making necessary adjustments in real-time.
• Foster strong relationships with patients and their caregivers by providing clear communication, education, and ongoing support.
• Conduct assessments that help patients achieve their specific functional and rehabilitation goals.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care and enhance treatment outcomes.
• Adhere to minimum utilization goals and contribute to the overall effectiveness of our home care service.

Patient Population / Client Focus

This role primarily serves patients transitioning from hospital settings or those seeking outpatient orthopedic rehabilitation. Your focus will be on individuals of diverse ages and backgrounds who require personalized care plans that cater to their unique needs and circumstances. Whether treating athletes recovering from injuries, children with developmental issues, or elderly patients with mobility challenges, your ability to connect with patients will be paramount in facilitating effective treatment and improving their functional abilities.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid state license to practice Physical Therapy in Virginia.
• A minimum of two years of experience in a clinical setting.
• Current BLS certification from the American Heart Association.
• Excellent interpersonal skills with a proven ability to build rapport with patients and their families.
• Strong written and verbal communication skills for effective documentation and patient education.
• Proficiency in using electronic medical record systems and basic computer software.

Preferred Experience

• Experience in home health settings or outpatient orthopedics is highly desirable.
