Work Environment

This position is primarily based in patients' homes, allowing for a unique and personalized approach to rehabilitation. Key Responsibilities

• Conduct thorough evaluations of patients recovering from injuries, surgeries, or chronic conditions, focusing on their specific orthopedic needs.
• Develop, implement, and adjust individualized treatment plans using evidence-based practices that may involve exercises, hands-on therapy, and modalities to reduce pain and enhance mobility.
• Monitor patients' progress through regular assessments, documenting changes and modifying treatment goals and plans as necessary.
• Educate patients and their families about orthopedic conditions and the therapeutic process, including home exercise techniques and injury prevention.
• Provide guidance on the selection and use of assistive devices, ensuring patients have the necessary tools to enhance their independence and quality of life.
• Collaborate with a multidisciplinary health care team to optimize patient care and contribute to comprehensive treatment strategies.
• Engage in continuing education and attend workshops to remain current within the field, enhancing personal expertise and contributing to the team's knowledge base.

Patient Population / Client Focus

Your focus will be on patients with orthopedic conditions, ranging from post-operative recoveries to chronic pain syndromes. The patient demographic is varied, including children and adults, each with distinctive needs and challenges. As part of our home care services, you will deliver therapy in a familiar environment, which often promotes faster recovery and greater comfort for patients.

Qualifications

• Bachelor's Degree in Physical Therapy is required; a Doctorate in Physical Therapy is preferred.
• A current state license to practice physical therapy is mandatory.
• Strong written and verbal communication skills are essential for patient education and team collaboration.
• Proficiency in using electronic health records and other software tools to document treatments and patient progress.
• Flexibility and adaptability to meet the varying needs of patients and work environments.

Preferred Experience

• Prior experience in an orthopedic clinical setting is advantageous but not required; we welcome recent graduates and will provide mentorship.

Compensation and Benefits
