Key Responsibilities\n

• Conduct comprehensive patient intakes, focusing on thorough diagnostic and functional evaluations to inform treatment plans.

• Collaborate closely with primary care providers to identify and manage psychosocial and mental health issues in patients.

• Provide brief, focused therapeutic interventions to patients seeking mental health services.
\n
• Initiate follow-up care to assess treatment effectiveness and make necessary adjustments to therapeutic strategies.

• Develop relapse prevention plans to help patients maintain stable emotional and mental health.
\n
• Detect at-risk patients and create strategies to prevent the worsening of their conditions.
\n
• Coordinate care among multiple health providers, ensuring comprehensive service delivery tailored to each patient's needs.

• Document patient progress and therapeutic interventions rigorously in clinical records.

• Provide educational materials and self-management skills to empower patients in their treatment journey.

• Consult with clinic management and team members on behavioral health services, contributing to program evaluations and improvements.

Patient Population / Client Focus\n

As a Licensed Professional Counselor at Yakima Family Health Services, you will engage with a diverse patient population that includes individuals from various cultural and socioeconomic backgrounds. Your focus will encompass emotional and mental health challenges among children, adolescents, and adults. Qualifications\n

• A minimum of a Master's degree in Clinical Counseling is required.

• Current and valid LPC (Licensed Professional Counselor) license.

• In-depth knowledge of mental health assessment, treatment planning, and approaches in psychotherapy.

• Strong capability in diagnosing mental health conditions using DSM-IV criteria.

• Excellent communication skills, with the ability to establish strong therapeutic alliances with patients, their families, and the primary care team.

• Experience in providing behavioral health care, including team collaboration.

Preferred Experience\n

Compensation and Benefits\n

The compensation for this position ranges from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
