Key Responsibilities\n

As a Licensed Master Social Worker at Tranquil Minds Wellness, you will:

• Conduct comprehensive assessments of seniors’ psychosocial needs, taking into account their emotional, mental, and social wellbeing.

• Work closely with clients and their families to create individualized care plans that align with their preferences and goals.

• Provide counseling and emotional support through teletherapy sessions, empowering seniors to cope with life's transitions such as health changes, loss, and family dynamics.

• Advocate for clients’ rights and access to community resources, ensuring they obtain the necessary services and support for a fulfilling life.

• Collaborate with a multidisciplinary team, including nurses and healthcare professionals, to deliver coordinated care and communicate effectively regarding client status.

• Conduct regular virtual check-ins to monitor clients' wellbeing and address any safety concerns or evolving needs.

• Facilitate family meetings to resolve conflicts and improve client engagement in their care process.

• Educate clients and families about community resources and support groups that can foster social connections and improve quality of life.

• Maintain meticulous records documenting assessments, care plans, and service delivery through electronic health record (EHR) systems.

Patient Population / Client Focus\n

You will primarily serve seniors, many of whom may be experiencing age-related challenges such as chronic illnesses, mental health difficulties, and social isolation. Your expertise will help guide them through these challenges, providing tailored support that enhances their overall wellbeing and integrates them positively into the community.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of experience as a social worker, preferably in a geriatric or community center setting.
