Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to understand the emotional, mental, and social needs of senior clients, ensuring all factors are taken into account.

• Collaborate closely with clients, their families, and interdisciplinary teams to create tailored care plans that resonate with each individual’s goals and lifestyle preferences.

• Provide expert counseling and emotional support to seniors and their families, aiding them through various life transitions such as health crises or moving to assisted living.

Patient Population / Client Focus\n

You will primarily serve senior clients, many of whom are navigating complex emotional challenges related to aging, health deterioration, and significant life changes. Your expertise will be instrumental in guiding these individuals and their families through emotionally charged situations, providing support that respects their dignity and autonomy.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid and current LCSW state licensure to practice social work.

• A minimum of two years’ experience in a social work capacity, with a preference for backgrounds in geriatric care or community health settings.

• In-depth understanding of the principles of social work, particularly as they apply to the older adult demographic, along with a solid familiarity with community resources.

Preferred Experience\n

Experience working in a detox or substance recovery setting is strongly preferred, as well as expertise in managing the complexities of mental health challenges specific to the aging population.

Compensation and Benefits\n

The annual salary for this role ranges from $70,000 to $100,000, commensurate with qualifications and experience.
