Work Environment\n

This position is designed for remote work, providing flexibility that allows you to balance your professional responsibilities with personal life. You will be working within the framework of a hospice center, collaborating closely with multi-disciplinary teams to ensure that clients receive high-quality, compassionate care tailored to their individual needs. While most of your engagement will be virtual, occasional visits to community partners or homes may be necessary for assessments or outreach activities.

Key Responsibilities\n

• Conduct comprehensive assessments to identify clients' social, emotional, and psychological needs, ensuring a holistic approach to care.

• Provide personalized individual and group counseling using evidence-based therapeutic approaches tailored to diverse needs.

• Develop and implement comprehensive service plans that address clients' specific goals and objectives, aiming for substantial improvement in their well-being.

• Advocate for clients' rights and access to necessary resources, emphasizing social justice and equality within community settings.

• Collaborate with community organizations, government agencies, and volunteers to enhance the reach and effectiveness of our services.

• Participate in ongoing program development and evaluation, and assist with grant reporting to secure funding for essential programs.

Patient Population / Client Focus\n

In this role, you will work with a diverse client population, including individuals and families who are experiencing significant life challenges, grief, or emotional distress related to terminal illness and bereavement. Your efforts will be crucial in facilitating coping strategies, providing emotional support, and ensuring clients have access to vital community resources that promote their overall quality of life.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

• Proven ability to conduct assessments and provide evidenced-based therapeutic services.

• Strong understanding of trauma-informed care principles and social justice advocacy.

• Excellent communication, empathy, and active listening skills, with a focus on building trust and rapport with clients.

Preferred Experience\n

Compensation and Benefits\n
