Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for patients and families upon admission, providing insight into their social, emotional, and physical needs through the application of social work theories and techniques.

• Work closely with multidisciplinary health care teams to create, implement, and refine patient and family psychosocial treatment plans, while also facilitating referrals to specialty services and social support systems.

• Provide ongoing supportive counseling, crisis intervention, and grief therapy, assisting individuals and families in navigating the emotional complexities associated with their health conditions.

• Maintain an independent caseload with varying clinical acuity, effectively balancing clients' diverse needs while ensuring quality of care and ethical practice.

• Engage in proactive case finding to identify patients and families who may benefit from psychosocial support, participate actively in team conferences, and contribute to interdisciplinary rounds.

• Educate patients and their families about adapting to diagnoses, treatments, and prognosis, guiding transitions from youth to adult services with a focus on reducing anxiety and enhancing understanding.

• Ensure accurate and timely completion of electronic medical records, adhering to organizational policies while providing thorough documentation through written and oral reports.

• Mentor new MSWs and interns by providing individual and group clinical supervision, sharing insights from personal experience to foster professional development.

Qualifications\n
