Key Responsibilities\n

• Conduct thorough assessments of clients’ psychosocial needs, identifying challenges and potential interventions.

• Collaborate with a multidisciplinary team to develop Individualized Family Service Plans (IFSP) that outline tailored interventions and support strategies.

• Regularly monitor and evaluate client outcomes, facilitating adjustments to care plans as necessary to optimize results.
\n
• Provide therapeutic support to clients and their families, guiding them through the complexities of aging, health issues, and life transitions.
\n
• Establish trusting relationships with seniors and their families, creating a positive and supportive atmosphere that encourages open dialogue.

• Ensure compliance with all Federal and State regulations governing social work practices, maintaining high standards of ethics and professionalism.

• Utilize clinical skills to administer evaluations and assessments, documenting findings thoroughly in client records.

• Engage in ongoing professional development to stay current with best practices in social work and mental health care.

Qualifications\n

• A Master's degree in Social Work (MSW) or a related field is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.

• Proven competency in diagnostics, crisis management, risk assessment, and developmentally appropriate interventions for seniors.

• Excellent verbal and written communication skills, with a demonstrated ability to build rapport with clients, families, and colleagues.

• Strong organizational skills and the ability to manage multiple cases simultaneously.

Preferred Experience\n

Compensation and Benefits\n
