Key Responsibilities\n

Reporting to the Social Work Manager, your key responsibilities will include:

• Conducting thorough bio-psychosocial evaluations to identify and address barriers to treatment, aiming to enhance patients' overall health and utilization of resources.

• Collaborating with medical and support staff to develop comprehensive care programs tailored to individual patient needs, ensuring coordinated care and improved treatment compliance.

• Developing and implementing treatment plans using evidence-based practices, providing individual and group therapy sessions while continually assessing and adjusting therapeutic approaches based on patient feedback.

• Documenting all social work activities and patient interactions accurately in medical records, adhering to departmental standards.

• Educating patients and families on the importance of medical and social work interventions to facilitate a smoother transition to optimal health and functioning.

• Identifying advocacy needs within the patient and family system, coordinating appropriate services and community resources to improve support networks.

• Completing case-related statistics and reports in accordance with departmental standards to track progress and outcomes.

• Maintaining an up-to-date knowledge of available community resources and making referrals as necessary.

• Engaging in continuous professional development and education, attending relevant training to enhance clinical skills.

Patient Population / Client Focus\n

As an LCSW at Sun Valley Consultation Services, you will primarily work with patients experiencing end-of-life issues and their families. Compensation and Benefits\n

This position offers a competitive salary range of USD 85,000 to 100,000 per year and includes a comprehensive benefits package.
