Key Responsibilities\n

• Conduct thorough clinical assessments of patients referred to our hospice center to understand their mental health and emotional needs.

• Develop and implement individualized treatment plans based on assessment results, incorporating evidence-based therapeutic interventions tailored to each patient.

• Utilize solution-based psychotherapy techniques to facilitate sessions aimed at promoting coping strategies and emotional well-being.

• Engage in regular collaboration with a multidisciplinary treatment team to ensure comprehensive patient care and continuity of services.

• Provide educational resources and support to patients and their families about mental health issues, treatment options, and coping mechanisms.

• Maintain accurate and timely clinical documentation including patient progress notes and treatment summaries using secure, HIPAA-compliant electronic health record systems.

• Actively participate in professional development by staying updated on current research, best practices, and advancements in the field of mental health.

Patient Population / Client Focus\n

You will work with a diverse group of patients experiencing various emotional and psychological challenges, often related to chronic illness and end-of-life care. This experience requires a deep understanding of the unique needs of individuals and families facing grief, loss, and transition, all while providing empathetic support to enhance their overall experience in the hospice setting.

Qualifications\n

• A Master's degree in Social Work from an accredited program is required.

• Possession of a valid LCSW license in the state of New York.

• Proficient knowledge of therapeutic interventions specifically geared towards solution-based psychotherapy.

• Exceptional interpersonal and communication skills to effectively interact with patients, families, and multidisciplinary teams.
