Key Responsibilities\n

• Conduct thorough assessments, diagnoses, and evaluations of patients to determine their mental health status and the effectiveness of current care interventions utilizing recognized metrics such as the DSM-5.

• Develop comprehensive, individualized treatment plans that consider cultural diversity and specific needs of each patient, focusing on achievable and measurable outcomes.

• Utilize evidence-based therapeutic modalities to support patients and families, continually assessing their progress while making necessary adaptations to the treatment approach.

• Ensure accurate and timely maintenance of clinical documentation and treatment records, meeting all regulatory and organizational standards.

• Collaborate with patients, their families, and interdisciplinary teams to refine treatment objectives and enhance overall care, taking proactive measures in patient safety and crisis intervention.

• Act as a liaison by coordinating referrals and working closely with external treatment providers and organizations, fostering strong professional relationships to streamline patient care.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited program.
