Key Responsibilities\n

As a Licensed Professional Counselor, your daily responsibilities will include:

• Conducting comprehensive assessments and ongoing evaluations to identify the needs of your patients and to formulate appropriate treatment plans.

• Communicating effectively with supervisors, direct care staff, family members, and other stakeholders to provide updates on patient progress and collaborative care efforts.

• Coordinating home visits and in-home observations as necessary to evaluate the patient's environment and context for their care.

• Ensuring the effective implementation of treatment plans and modifying them as indicated by patient progress or setbacks.

• Participating in interdisciplinary team meetings to discuss and optimize patient outcomes in a collaborative framework.

• Documenting detailed monthly and quarterly progress notes to track therapeutic developments and adjustments.
\n
• Providing ongoing follow-up with patients through interviews or additional home visits to ensure continuous support.
\n
• Maintaining organized and accessible patient records for operational transparency and ease of information sharing with authorized parties.

• Acting as a liaison with the wider mental health community to enhance programmatic goals and inter-agency collaboration.

Patient Population / Client Focus\n

You will work with a varied patient demographic, including adults and youth dealing with a range of mental health challenges. We're particularly focused on serving individuals and families grappling with issues such as anxiety, depression, trauma, substance misuse, and relationship conflicts. The treatment approach emphasizes personalized care to foster sustainable recovery and empowerment.

Qualifications\n

We are looking for candidates who meet the following criteria:

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in Arizona.

• A minimum of 2 years of counseling experience in a clinical setting.

Preferred Experience\n

Compensation and Benefits\n
